Title Therapist

Description Conducts Family-centered strengths based individualized intakes / assessments gathering information from family, consumer, significant other and involved agencies in the field or in the office as appropriate.
Complete Service and Coordination Plans, progress notes, and other DMH required documentation in a timely manner.
Complete all internal Pacific Clinics paperwork within 48 hours of service delivery.
Provide rehabilitative services to clients and/or their families which might include assistance in restoring or maintaining a client’s functional skills, daily living skills, social skills, medication compliance, and linking up to needed community services and support resources.
Provide outreach, advocacy, and rehabilitative services needed for ongoing cases and in crisis intervention.
Provide case management, which might include obtaining client information, identifying needs and problems, linking clients and their families to appropriate resources.
Attend and participate in training program, inter-agency case conferences, and IEP meetings, as appropriate.
Work with the treatment team to provide appropriate coordinated care services to clients and their families.
Work on a multicultural team sensitive to the cultural and linguistic needs of the clients and families served.
Perform other duties as assigned by the Team Leader, Program Director or his/her designee.
Participates in after hours on-call courage as part of the on-call rotating schedule.
Program Development and Training
Attend and participate in staff meetings to provide input towards program development and staff training. Interacts with other internal programs, clinical/support staff and school personnel.
Models Pacific Clinics’ approach, mission and core values in all communication and correspondence.
Documentation
Document clinical charts in a manner that is timely and meets the standards of the Department of Mental Health Short/Doyle Medi-Cal, Pacific Clinics and its funding sources.

Requirements Licensed or license-eligible LCSW, MFT or Psychologist and 2 years experience working with severely emotionally disturbed children in a day treatment or outpatient setting. State waiver may be required for licensed-eligible registered psychologists, excellent communication skills, and demonstrated ability to relate to a culturally diverse clientele and staff. Valid CA driver license and insurability on the Clinics' auto-liability insurance policy. Spanish speaking preferred.
