Encourages clients to meet needs through matching strengths with tasks.

Assesses systems effectiveness in meeting the individual needs of each client.

Works in conjunction with community providers to assist clients in meeting goals.

Connects the client and family to community resources maximizing information from the strengths-based assessment.

Provides field contacts at home, school, and the community to promote stabilization and further support.

Under the direct supervision of the Program Director, assists in the identification of triggers to escalation, and provides suggestions to the treatment team.

Collaborates, as a member of the treatment team to address the client needs advocating for stabilizing services and connecting the consumer to beneficial resources within the community.

Supports medication compliance.

Works as part of a multicultural team sensitive to the cultural and linguistic needs of the consumer/family.

Documents all services for Short-Doyle/Medi-Cal/EPSDT pursuant to funding source directives, Pacific Clinics’ protocol and HIPAA compliance requirements.

Completes all documentation within the timelines established by the Program for service delivery.

Models Pacific Clinics’ approach, mission and core values in all communication and correspondence.

Promotes an environment that is strengths-based and family-centered.

Completes all charting and paperwork within 48 hours of service delivery.

Requirements Experience working with SED children and families. Performs paraprofessional duties. Must demonstrate ability to work as a multidisciplinary team. Valid CA driver license and insurability on the Clinics' auto-liability insurance policy. Bilingual and Spanish preferred.
