Description POSITION SUMMARY: Provides case management to individuals with severe, chronic mental illness, coordinate services to insure that the members receive the maximum benefits from all interventions and develop social, self-help, educational and vocational resources, establish and maintain living arrangements, access healthcare, increase productive social contacts and engage in constructive activities.

PRIMARY RESPONSIBILITIES:
• Manages a caseload of assigned members with psychiatric disabilities.
• Formulates and implements a plan of treatment and rehabilitation which promotes member participation in the development and maintenance of the member’s individualized goals which are designed to facilitate individual growth, strengthen independent living skills, develop social interaction skills, and improve their daily living skills.
• Provides, refers, and/or links members or member's family or significant other to support services as needed. This may include, but is not limited to: mental health and co-occurring substance abuse rehabilitation, coordination of healthcare needs, vocational, education and socialization, housing, money management, and community integration.
• Closely monitors linkages to ensure that required services are provided.
• Assesses and monitors condition of member to evaluate success of treatment and adapts interventions as needed.
• Encourages family members/significant others to be active partners in the treatment plan.
• Collaborates with other community agencies and resources in the provision of services.
• Ensures all services are within Portals best practice guidelines and is aligned with the Psychosocial Rehabilitation Model.
• Document all services provided and maintain appropriate productivity standards.
• Counsels members individually and in-group sessions to assist members in coping with mental illness, and/or alcohol or other drug abuse problems.
• Leads groups and activities to develop and enhance member functioning in the community.
• Prepares documents for presentation in court, accompanies member to court.
• Accompanies members to appointments for treatment, hearings, and other necessary appointments.

Documentation:
Document clinical charts in a manner that is timely and meets the standards of the Department of Mental Health Short/Doyle Medi-Cal, Pacific Clinics and its funding sources. Document clinical charts in a manner that
Requirements BA in Psychology, Social Work or related field. Prefer minimum two (2) years of experience working with persons with severe chronic mental illness. Excellent written and verbal communication skills. Demonstrated experience and ability to relate to a culturally diverse clientele and staff Valid CA driver license and insurability on the Clinics' auto-liability insurance policy. Bilingual Spanish preferred but not required.
